Back Pain Red Flags: When It Is More Than Muscle Strain

The Vast Majority of Back Pain Is Benign

95% of back pain is non-specific β€” caused by muscle strain, poor posture, or degenerative changes β€” and resolves within 4–6 weeks. However, a small but important minority of back pain cases signal serious underlying conditions. Knowing the red flags can save your life.

Back Pain Red Flags

Cauda Equina Syndrome (Emergency)

  • Bladder dysfunction: urinary retention (inability to pass urine) or incontinence
  • Bowel incontinence
  • Saddle anaesthesia (numbness in inner thighs, buttocks, genitals)
  • Bilateral leg weakness
  • ⚠️ This is a surgical emergency β€” call 112 or go to ER immediately. Irreversible paralysis occurs without surgery within hours.

Malignancy Red Flags

  • Age >50 with new back pain
  • Known cancer history
  • Unexplained weight loss
  • Night pain β€” wakes you from sleep (not positional)
  • Pain at rest, not relieved by lying down

Infection / Abscess

  • Fever with back pain
  • Recent spinal procedure or IV drug use
  • Immunocompromised (HIV, diabetes, steroids)
  • Tenderness to percussion over vertebrae

Fracture Red Flags

  • Osteoporosis in elderly with sudden severe back pain after trivial activity
  • Major trauma at any age
  • Steroid use (high fracture risk)

Should I get an MRI for back pain?

MRI is NOT needed for typical non-specific back pain <6 weeks without red flags. Early MRI often shows "abnormalities" (disc bulges) present in asymptomatic people, leading to unnecessary anxiety and procedures. If red flags are present, MRI should be done urgently.
