Craniovertebral Junction Disorders: A Comprehensive Guide

What Is the Craniovertebral Junction?

The craniovertebral junction (CVJ) is the highly specialised junction between the skull base and the upper two cervical vertebrae (C1/Atlas and C2/Axis). It allows the greatest range of head movement in the spine. Structural abnormalities here can compress the brainstem and upper spinal cord β€” causing serious neurological dysfunction.

Common CVJ Disorders

Basilar Invagination (BI)

The tip of the C2 vertebra (odontoid process) migrates upward into the skull base, compressing the brainstem. Common in India β€” often associated with atlanto-axial instability and Chiari malformation.

Atlanto-Axial Instability (AAI)

Excessive movement at the C1-C2 joint due to ligamentous laxity (Down syndrome, rheumatoid arthritis) or trauma. Can cause sudden catastrophic spinal cord injury even from minor falls.

Chiari Malformation

Herniation of cerebellar tonsils through the foramen magnum into the spinal canal. Causes headache (worsened by coughing/straining), neck pain, arm numbness, and in severe cases syringomyelia (fluid cavity in the spinal cord).

Diagnosis

  • MRI brain and cervical spine (FIESTA/CISS sequences)
  • CT with 3D reconstruction for bony anatomy
  • Dynamic X-rays (flexion-extension) for instability
