Understanding Ankle Sprains
Ankle sprains occur when the ankle rolls or twists, overstretching or tearing the ligaments. They are the most common musculoskeletal injury β accounting for 25% of all sports injuries. The lateral ligaments (ATFL, CFL) are most commonly affected.
Grades of Ankle Sprain
- Grade 1 (mild): Microscopic ligament tears; mild swelling and tenderness; full weight-bearing possible
- Grade 2 (moderate): Partial ligament tear; moderate swelling and bruising; painful weight-bearing
- Grade 3 (severe): Complete ligament rupture; significant swelling, bruising, instability; unable to bear weight
Immediate Treatment (RICE)
- Rest β avoid weight-bearing for 24β48 hours
- Ice β 20 minutes every 2β3 hours
- Compression β elastic bandage
- Elevation β ankle above heart level
Rehabilitation (Critical to Prevent Re-injury)
- Phase 1 (0β7 days): Pain and swelling control
- Phase 2 (1β3 weeks): Range of motion and strengthening
- Phase 3 (3β6 weeks): Balance and proprioception training (wobble board)
- Phase 4: Sport-specific training before return to play

How do I know if my ankle is broken or sprained?
The Ottawa Ankle Rules help: if there is bone tenderness at the tip of either malleolus, or inability to take 4 steps, an X-ray is needed to rule out fracture.
Why do ankle sprains keep recurring?
Recurrent sprains are due to undertreated initial injury β inadequate proprioception training leaves the ankle unstable. Bracing and balance exercises significantly reduce recurrence.
