Hip Pain in Your 30s & 40s: Should You Be Concerned?

Hip Pain Is Not Just for the Elderly
While hip arthritis is common after 60, hip pain in younger adults (30sβ40s) often has distinct causes that are treatable β and important not to miss. Early diagnosis prevents minor issues from progressing to joint destruction.
Common Causes in Young Adults
Femoroacetabular Impingement (FAI)
Abnormal bony bumps on the femoral head or acetabulum cause friction during movement, damaging the labrum and cartilage. Presents as groin pain with flexion and rotation. Arthroscopic surgery is highly effective.
Labral Tear
The cartilage ring (labrum) around the hip socket can tear from FAI, trauma, or hypermobility. Causes deep groin pain, clicking, and a "giving way" sensation. MRI arthrogram confirms diagnosis.
Avascular Necrosis (AVN)
Blood supply to the femoral head is disrupted, causing bone death. Risk factors: long-term steroids, alcohol, sickle cell disease, trauma. Early: MRI diagnosis; Advanced: core decompression or joint replacement.
Stress Fracture
Common in runners and military recruits β groin pain worsened by activity, relieved by rest. MRI is the gold-standard diagnostic. Requires immediate non-weight-bearing.
What are red flag hip pain symptoms?
Night pain, rest pain, rapid-onset severe pain, fever with joint pain, pain in a cancer patient β these require urgent evaluation. Do not assume hip pain in a young person is "muscular".
