Types of Ear Infections in Children
Acute Otitis Media (AOM)
Middle ear infection β the most common childhood bacterial infection. Peaks at 6β18 months. Bacteria (Streptococcus pneumoniae, H. influenzae) infect the middle ear fluid, causing rapid-onset ear pain, fever, and fussiness.
Otitis Media with Effusion (Glue Ear)
Fluid accumulation in the middle ear WITHOUT acute infection. Common after AOM or with enlarged adenoids. Often causes mild hearing loss β may affect speech development if persistent. Often resolves spontaneously in 3 months.
Otitis Externa (Swimmer's Ear)
Infection of the ear canal skin β caused by water trapping (swimming pools) and Pseudomonas. Painful ear (especially on tugging), discharge, itching. Treated with antibiotic ear drops.
Recognising Ear Infection in Infants
- Unexplained fever
- Tugging or pulling at ear
- Crying more than usual, especially when lying down
- Trouble sleeping
- Difficulty hearing (does not respond to sounds)
- Fluid draining from ear (if eardrum has perforated)
Treatment
- Pain relief: paracetamol or ibuprofen
- Antibiotics: amoxicillin 7β10 days (for children under 2, or severe symptoms; watchful waiting acceptable for older mild cases)
- Glue ear: decongestants not effective; consider ENT referral for grommets if hearing loss persists 3+ months
Do all ear infections need antibiotics?
Not in older children with mild symptoms β 60β80% of AOM resolves without antibiotics. "Watchful waiting" for 48β72 hours is appropriate for children over 2 with mild AOM. Antibiotics are always needed for children under 6 months, severe pain, or both ears affected.
