Is it Asthma? Common Symptoms
- Frequent or recurrent wheezing (high-pitched breathing sound)
- Persistent cough – especially at night or with exercise
- Shortness of breath
- Chest tightness
- Symptoms triggered by exercise, cold air, or infections
- Symptoms improved with bronchodilators (reliever inhaler)
Common Triggers
- Respiratory infections (viruses) – most common trigger in children
- Air pollution and dust – significant in Delhi
- Allergens: Dust mites, pet dander, cockroach, mould
- Exercise-induced bronchoconstriction
- Tobacco smoke: Second-hand smoke
- Cold air and sudden weather changes
- Strong emotions: Crying, laughing
Diagnosis
Clinical history and examination. Spirometry (>5 years) showing reversible airway obstruction. Peak flow monitoring. Allergy testing. Trial of bronchodilator therapy.
Asthma Treatment
Reliever Medications (SABA)
- Short-Acting Beta-Agonists: Salbutamol (Ventolin) inhaler – used for acute symptoms
- Should not be needed more than 2 days/week
Controller Medications
- Inhaled Corticosteroids (ICS): First-line preventer – Fluticasone, Budesonide
- LABA + ICS: For moderate-severe asthma
- Leukotriene Receptor Antagonists: Montelukast – particularly useful in Delhi pollution context
- Biological therapies: For severe allergic asthma (omalizumab)
Delivery Devices
- Spacer with MDI: Best for children under 5 years
- Dry Powder Inhaler: Children 6 years and older
- Nebuliser: For acute attacks
Asthma Action Plan
