Fever in Children – What it Means

Fever is one of the most common reasons parents bring children to the doctor. Fever itself is not a disease – it is the body’s natural response to infection. However, it can be distressing for parents and sometimes indicates a serious underlying condition. What Temperature is Fever?

  • Normal temperature: 36.5°C – 37.5°C (97.7°F – 99.5°F)
  • Low-grade fever: 37.5°C – 38°C
  • Fever: ≥38°C (100.4°F)
  • High fever: ≥39°C (102.2°F)
  • Very high/dangerous fever: ≥40°C (104°F)

Common Causes of Fever in Children

  • Viral infections (most common): common cold, flu, roseola, hand-foot-mouth disease
  • Ear infections (otitis media)
  • Throat infections (tonsillitis, strep throat)
  • Urinary tract infections
  • Pneumonia
  • Dengue, malaria (important in Delhi)
  • Post-vaccination fever (normal and expected)

Fever Management at Home

  • Paracetamol (15 mg/kg/dose every 4–6 hours) – safest first choice
  • Ibuprofen (>6 months age, 10 mg/kg/dose) – alternative
  • Dress child lightly, maintain room temperature
  • Encourage fluids – breastmilk, ORS, water
  • Tepid sponging for comfort (NOT ice/cold water)
  • Monitor for warning signs

When to See a Doctor Urgently

  • Any fever in a child under 3 months
  • Temperature above 40°C
  • Fever lasting more than 3 days
  • Child appears very unwell, limp, or unusually drowsy
  • Difficulty breathing
  • Rash with fever
  • Seizure with fever
  • Persistent vomiting preventing medications
